issue with series and sqrt()

  I have a simplified example attached that shows that the series of a sqrt() with argument consisting of a sum of terms return a wrong result. In the attached notebook one can see one term in the second order is missing. What am I doing wrong? Thank you for any tips.., -- Philippe.
