application error

Configuration Information:
Operating system: Windows NT 4.0

I can not attach a file to my faxes.....every time I attempt to
attach a file I receive a dialoge box saying there has been an
application error and the whole program crashes. It was working fine
for months then all of a sudden this.....HELP.....note: it seemed to
mess up when I was moving back and forth trying to edit or replace
attachments for a particular fax.
