Custom Contact Form in Outlook + Intellisync Problems

I was hoping someone might be able to help me out... I have created a custom
contact form in Outlook but when I click on FIELD MAPPING in intellisync
v3.7, it crashes. I've tried other versions of intellisync and same thing
keeps happening.
