Hi Lisa:

With the CD ROm, you shouldn't need a start-up disk since you can
boot from the CD ROm. if you still need a start-up disk, you can use
the disk images on the CD ROm as described in this Knowledge Base
link:

Alternately, In order to create a start-up disk follow the following
instructions by following this link to an article in the Knowledge
Base about using Start-up disk builder. It will require that you
have a copy of the Apple disk Tools disk that came with your system
software:
