407 Error when Uploading for the First Time

After restarting a computer and using SWFUpload behind an authentication-required proxy, uploads return a 407 (Proxy authentication required) error; no username/password dialog is shown. Refreshing the page and trying the upload again works without any issues. Any subsequent uploads (different browser, restarting browser, clearing cache, etc) works until the computer is restarted again.

Any ideas what might be happening here? Why does only the first upload not work?

What kind of browser are you using?
