What is the file format for point source monthly loading in ArcSWAT

Hi everyone!
I have a point source in the domain of my model. When I edit point source inputs, I am asked to input a *.txt/*.dbf as the average monthly daily loading file. I have no idea of the format of this file, and didn't find it in the mannual. Could you please show me how to write this file?
