SWAT+ Check fails, "Unable to get SWAT+ Check data from database"

Dear SWAT+ team,

SWAT+ Check returns "Unable to get SWAT+ Check data from database", although the model runs and the output analysis completes normally.

SWAT+ Editor 4.0.0, SWAT+ rev. 62 (official), Windows 10.
Already tried: re-running the analysis and rebuilding swatplus_output.sqlite (with and without "only read files required by SWAT+ Check"). No change.
The average-annual tables exist and are populated.
Likely cause: in channel_sd_aa / basin_sd_cha_aa, three columns appear as undefined1/2/3 (named null in the output files, plus a water_temp column), so the channel format from rev. 62 seems not to match what SWAT+ Check in Editor 4.0.0 expects.

Is this a known rev. 62 / Editor 4.0.0 mismatch, and what is the recommended fix?
