CTK SVDP - reminders on Ozanam requests

I have been getting reminders from the district regarding Ozanam.  They keep adding more little items.   We have been very good at meeting them.   I will have this all on the June agenda, but wanted to send it out.

I would also like to remind you that the Conference Support team has two business days to send you the request approval notification after receiving the application with all complete documentation via Serware. But this email should be addressed to Oza...@ssvpomaha.org, rather than being sent individually to Kristina or Lily's email addresses.

The documents needed to process a rental assistance request are:

*   Lease agreement

*   Tenant ledger

*   Eviction notice

*   Signed and dated W-9 form

*   Landlord's name, email address, phone number, and confirmation of the address where the check is to be mailed.

Utilities:

*  Shut off Notice

 

Last but not least, I would like to remind you that the information in the neighbors' profiles must, as a mandatory requirement, include their date of birth; it is also highly desirable to include their email address and phone number, if available.

 

This is the format I try to use:

Ozanam request for $xxxx  (max of $750)

Neighbor needing the funds - indicate what the crisis is, that they have not received Ozanam and have not been helped in the last 12 months.   Add any other items such as what funds are we providing if Ozanam is approved, etc. 

Indicate a note indicating that the documents are in the neighbor file location (I also want the documents in the assistance request file location)

Landlord information - email as a minimum.
