This is my last notice to you. You still have not completed the Health and Liability survey that I sent out several times since June.

This is SERIOUS that I have this on file with me on event weekend.  If I do not receive an email from you that you have completed this and if your name does not show up on my print out, I will not give your Send Off Packet/Event Jersey and or I will pull your Event packet in Tahoe.  Should something happen to you, I have no information for police, fire or paramedics.

 

I don’t care if you have filled this out in the past, its oblivious that I don’t have it.
