Recent military service and medical records are not online. However, most veterans and their next of kin can obtain free copies of their DD Form 214 (Report of Separation) and the following military service records any of the ways listed below.
There is a fee for records that are considered "Archival," which depends on the discharge date. If the request is made 62 years after the service member's separation from the military, the records are now open to the public and subject to the public fee schedule (44 USC 2116c and 44 USC 2307). Response times from NPRC vary depending on the complexity of your request, the availability of the records, and our workload. We work actively to respond to each request in a timely fashion, keep in mind we receive approximately 4,000 - 5,000 requests per day.

You can mail or fax your signed and dated request to the National Archives' National Personnel Record Center (NPRC). Be sure to use the address specified (either in the instructions on the SF-180 or in our online system, eVetRecs). Most, but not all records, are stored at the NPRC. Special Note on Contacting by Email: Requests for military personnel records or information from them cannot be accepted by email at this time. The Privacy Act of 1974 (5 U.S.C. 552a) and Department of Defense directives require a written request, signed and dated, to access information from military personnel records. Yes, you can review your records even if you do not wish to get copies at that time. You would contact the Release of Information section of the HIM/Medical Records department to schedule an on site appointment. Please keep in mind the Release or HIM/Medical Records staff would not be able to answer clinical questions. Should you have clinical questions, please make us aware at the time you schedule the appointment so that we can make an attempt to get you in touch with the appropriate clinical staff.
To obtain a copy of a medical record from Memorial Healthcare System, you must complete and submit the Authorization for Release of Confidential Medical Records Form. This is a Memorial policy, in accordance with federal and state laws, to protect the privacy and confidentiality of our patients' personal medical record information. Completion of this form is required to received a copy of your medical records or to have the medical record sent by Memorial to another party, such as a physician or other health care service provider, an insurance company or an attorney.

Vanderbilt University Medical Center has turned over transgender patient medical records to the Tennessee Attorney General's office, which confirmed Tuesday it is conducting an investigation into potential medical billing fraud.
The Tennessean reviewed a VUMC notice informing patients of the move, which the facility said was the result of an investigation into "billing for transgender care services provided to individuals enrolled in State-sponsored insurance plans." The state requested medical records from Jan. 1, 2018 to the present.
The state has legal standing to seek the private medical records, per a health care legal expert. However, the move has sparked privacy concerns among families whose children sought treatment at VUMC amid an increasingly contentious political climate surrounding transgender issues in Tennessee, where multiple legal battles are ongoing over state policies.
"VUMC received requests from the Office of the Tennessee Attorney General as part of its investigation seeking information about transgender care at VUMC. The Tennessee Attorney General has legal authority in an investigation to require that VUMC provide complete copies of patient medical records that are relevant to its investigation. VUMC was obligated to comply and did so," John Howser, VUMC's chief communications officer, said in a statement.
Court records filed in a separate case reveal Tennessee officials used a powerful legal tool called a civil investigative demand, which doesn't require probable cause or a judge's sign-off to compel testimony or documents. Tennessee first sought the medical records for a list of specific patients, before issuing another CID in March with sweeping demands for broad documentation, including emails sent and received from a general LGBTQ health program email address and the names of any individuals referred to the transgender clinic. It's unclear if Vanderbilt has supplied those records to the state.
Paul Hales, a St. Louis-based attorney who specializes in health information privacy, said the attorney general has legal recourse under federal and state law to request patient health records if it is in fact investigating a possible issue at the medical center.
