Here is the link to the San Francisco health plan, Healthy San Francisco.  It doesn't provide everything but it does cover basics. 

Healthy SF Chinese language website

Sunny should be able to qualify as the required documentation is fairly minimal.  All that is needed is something like a utility bill, drivers' license, bank statement, etc. listing a San Francisco address.

In general San Francisco is probably the best place to be for people with disabilities.  Even SF State has a department dedicated to these issues.  http://www.sfsu.edu/~dprc/staff.html  http://bss.sfsu.edu/disability/
