It's been a while since I am not following the mailing list, so forgive me if it's an already asked question.

I saw that Subsurface uses QT6, that should automatically adapt to HiDPI, but I can't make it recognize 4K on Windows 11.

Apart from forcing it from Compatibility panel and have blurred icons, is there something I am missing?

When sending things like this it always helps to tell us which version you are using,  and for this particular question,  which build.
The default binaries are still built with Qt  5... but there are Qt 6 binaries available... at least for the latest versions.

Subsurface x64 6.0.5661-CICD-release

As far as I can see in the folder, it's QT5 version. Where can I find the QT6 one?

That's such a great question. Because it appears we have "lost" those builds in the daily updates from CICD.
I need to look into this to figure out where they went - but that should be easy to fix.
