Background: Lack of eyecare protective measures especially in unconscious and sedated critically ill patients, make them prone to ocular surface diseases (OSDs), e.g., exposure keratopathy. This study is aimed to frame an algorithm-based approach to eyecare via eyecare bundle to bring down the burden of OSDs in critically ill patients especially in resource-limited settings.
Materials and methods: After clearance from institutional ethical committee, a quasi-experimental single center study was conducted over a period of 6 months. Incidence of exposure keratopathy was calculated before and after induction of eyecare bundle and was compared. Statistical analysis was done using SPSS software v20. p-value of less than 0.05 was considered significant.
Results: A total of 218 patients were enrolled in the study after obtaining informed written consent and after fulfilling inclusion criteria. Patients were divided into control and experimental groups, with baseline characteristics similar in both the groups, respectively, in terms of gender, age (40 years), APACHE II score, and specialty distribution except predominantly medical patients in experimental group. In control group (n = 99), total 69 patients (41 medical and 28 surgical) developed exposure keratopathy, while in experimental group (n = 109) only 15 patients (6 medical and 9 surgical) developed exposure keratopathy, hence a significant reduction was observed. Further follow-up of patients in the experimental group was also done on Days 5 and 7, respectively.
Conclusion: The proposed protocolized algorithm-based eyecare bundle significantly reduced the incidence of exposure keratopathy in sedated, mechanically ventilated, and vulnerable critically ill patients.

Estimating Cedar Shingles:
1. Start by measuring the amount of square footage you have to cover.
2. Each bundle will vary in the amount of completed coverage it will yield based on the width of exposure you choose. Exposure is the height in inches of the shingle face not covered by the next shingle course. Generally, the wider the exposure, the more coverage you will get from the bundle.
3. Using the chart above, identify the amount of coverage per bundle at your desired exposure.
4. Divide the square footage of coverage needed by the square footage per bundle value that you just selected from the chart.
Example: 500 square feet of wall needs to be covered using #1 Grade 18" (Perfection) Cedar Sidewall Shingles. The highest level of exposure is desired; therefore, the shingles can be installed as a double course with a 14" exposure. At 14" exposure, each bundle of shingles should yield 64 square feet of coverage. So, divide the 500 square feet of wall coverage by the 64 square feet of coverage per bundle (500 / 64 = 7.8 bundles). Since the calculation indicates that a partial bundle is required, round up to 8 bundles to ensure that enough shingles will be available to complete the job.

We previously proposed that PKHD1L1 may play a role in establishing connection between the OHC stereocilia with the tectorial membrane. The late-onset mild hearing loss phenotype, and increased DPOAE thresholds, observed in PKHD1L1-deficient mice is similar to that of previously reported in CEACAM16-deficient mice with TM deficits (Goodyear et al., 2019). Furthermore, PKHD1L1 is primarily located on OHC, towards the tips of the stereocilia (Fig. 3)(Wu et al., 2019), where TM attachment crowns form. Here, however, we present data that suggests the role of PKHD1L1 is neither (1) critical to, or (2) limited to, TM attachment crown formation.
PKHD1L1-difficent mice are sensitive to moderate acoustic overexposure (Fig. 7). Curiously, following noise damage, stereocilia loss and disorganization in PHD1L1-deficient mice was observed in tall row stereocilia (Fig. 8). Tall row damage after noise exposure in PKHD1L1-deficient mice could result from aberrant interaction between the stereocilia and tectorial membrane, or an inability of the bundle to cope with the potential increased strain on the stereocilia during noise trauma. Intriguingly in the Tubby mutant tub/tub;Map1aAKR mice, attachment crowns are present in the absence of horizontal top connectors. These mice are deficient in the attachment crown and horizontal top connector component TUB, combined with the restorative Map1a polymorphism on an AKR/N background. They display normal DPOAEs, while also exhibiting an increased sensitivity to noise damage (Han et al., 2020). As PKHD1L1-difficient mice display elevated DPOAE thresholds (Fig. 6 and Sup. Fig. 5) in addition to increased sensitivity to noise damage (Fig. 7), the loss of horizontal top connectors alone would not account for the phenotypes observed in these mice. However, the role of PKHD1L1 in the maintenance of stereocilia links, beyond attachment crowns, established during early postnatal development and often important for bundle cohesion (Goodyear et al., 2005), is worth further investigation.
