Students For Barrier-free Access Questionnaire

Dear Students,
I am a fourth year student studying Women and Gender Studies and Equity Studies (Disability Studies stream) at UofT. I am conducting a needs assessment of Students For Barrier-free Access (SFBA), a disability student organization, and am looking for participants to fill out a brief self-administered questionnaire. I am conducting this pilot research study partially on behalf of SFBA but also to fulfill a course requirement. The purpose of this pilot research project is 1) to find out what kinds of support students with disabilities on the University of Toronto campus need and want and 2) to assist the center in future program planning to better meet the needs of students on this campus. I am especially interested in finding out about those aspects of university life beyond academic and classroom experiences, including social, extra-curricular, and community involvement.
If you identify as a student with a disability at the University of Toronto and/or are registered with Accessibility Services on the St. George Campus, please fill out the attached survey and return the questionnaire via email. I am also happy to administer the survey orally, either in person or by telephone if the format provided is not suitable. All responses will be kept confidential. A final report summarizing the results of the questionnaire will be given to SFBA so that your feedback can be used to enhance future programming.
Please complete the questionnaire attached and return it to me by Monday July 28, 2008. If this email does not contain the questionnaire as an attachment, please let me know and I will forward you a copy directly.
