OpenOffice heading numbering

I'm in the process of putting a 100 page document together in OO2.0 and I'm
having trouble with heading numbers restarting. I googled and found a lot
of references "how to start renumbering", but I have the opposite problem.

I'm using the provided heading styles and numbering schemes - I think I made
no customizations. And most of the headings number quite nice like
1. first header on level 1
1.1. first header of level 2
1.2 second header of level 2
etc....

But at a certain point in time, the numbering restarted from scratch, no
matter where in the doc I insert a new heading, and the new series of
numbers keeps running quit consistenly between the earlier ones.

Anybody having a clue how I can get rid of this, without recurring to create
a new doc and cut and paste the contents of the first one?
