All chapter headings but one resemble this:

One chapter heading looks like this:

My question is whether "The Narrative of John Lexman" should be treated as <h3> or something else.

Sorry, somehow I missed this assignment.

The cover looks good. I would treat the chapter heading as part of the title, so yes wrapped in <h3> inside an <hgroup>.
