Some chapter headings have a <br/> element within the <h4> tags, to set off two parts of the title on separate lines.

When I build titles, it concatenates the text without the br tag, so it all runs together. For ToC and title, could I put an emdash between the parts? Or is there a better way to structure it?

That doesn’t feel like a place for a <br> as it’s effectively a subtitle. I’d go for:

build-title will then insert another colon between the title and subtitle.

Since the work is divided into book parts, I actually have h2 for the
"Book I" etc., and then chapters are h3, h4, with subtitle then
falling into h5. With build-title script, it leaves off the h5
subtitle. I suppose I could add it back in manually with a colon in
the <title> text. Also in the ToC, it uses an abbreviated form:

XI. Providence von Rosen: Act I: She Beguiles the Baron (in place of
"Act the First")
XII. Providence von Rosen: Act II: She Informs the Prince (in place
of "Act the Second")
etc.

so could I manually edit those <title> texts to match scan's ToC, then
when I build ToC, it should take what is in <title>, I think? There
are only four chapters like this.

After experimenting with the scripts, I'll just let build-title do its thing. The build-toc takes the text directly from h3 (roman ordinal) and h4 (title) from each file. Then it would just be an editorial decision whether to manually change ToC to match the abbreviated text like in the scans, including the subtitle.

build-title does not include subtitles in <title> or ToCs for chapters.
As long as your semantics are correct, build-title will do the right
thing every time.

There are a few forms of address with foreign terms, that only occur
in dialogue:

Herr Cancellarius (Latin: Chancellor), i.e. Mr. Chancellor
Herr Oberst (German: Colonel)
Frau Gräfin (German: Countess)

"Herr" and "Frau" are in M-W, but the others are not. Should I
italicize only the second part of the title as foreign? Or just stick
the second part in <span> tags with language? Or something else?

I don’t think I’ve come across this before, but my inclination would be that italicisation would make this look odd, so I’d go for the span treatment you suggested.

Relating to the hairspaces from typogrify, I'm getting stuck in a lint
loop. If I accept the typogrify changes, lint throws an error about
unexpected value for <title> element (s-021 error.) I think with the
ellipses starting the chapter title, typogrify breaks the algorithm
that calculates the title string from <h4>.

If I run build-title, it will fix that but then throw an error that
ToC does not match. (m-045 error.) So then if I build-toc, now lint
passes...but I think these build steps reverted the typogrify changes,
so I'm back to where I started! This is chapter 2-5. Should I take
the typogrify changes and ignore the lint error, or go another
direction with this?

OK, looks very good thanks! I've gone ahead and released it.

Regarding the typogrify issue in the title, basically you should do what
build-title does, which always correct, and ignore any more typogrify
changes. English typography is complex and not regular so we don't
expect typogrify to return a clean ebook every single time. It's OK for
it to suggest wrong changes, as long as it's already been run once.
