Good Afternoon,
We need to collect some information from all our affiliated clubs to provide to our insurers . For most clubs this should be reasonably straightforward but if you have any issues please let us know ASAP.
The deadline for completing this form is Tuesday 25th October. Apologies this is short notice, but it should only take a couple of minutes.
