TITLE: Service Connector – Children and Family Specialist
REPORTS TO: Senior Project Manager, Urban Strategies
JOB
SUMMARY: Responsible for identifying, providing and coordinating a
variety of supportive services and self-sufficiency opportunities for
children, youth and families living in HOPE SF projects
BACKGROUND:
HOPE
SF programs will involve the redevelopment of former public housing
sites as mixed income housing developments. As part of the human
services component of the HOPE SF program for the Hunters View
community, Urban Strategies is coordinating an integrated system of
supportive services and opportunities designed to help residents who are
vitally interested in overcoming barriers, increasing their income and
building self sufficiency. The purpose of the Hunters View Community
Support Program is to implment innovative and cutting edge strategies in
working with community and public partners to increase the human
capital-building assets in the revitalized Hunters View community.
The
Service Connector – Children and Family Specialist will be an integral
part of the HOPE SF staff team that will: 1) aggressively recruit
residents to participate in the Hunters View Service Connection Program
and perform initial and on-going assessments of clients; 2) work to
prepare clients to move toward housing self-sufficiency and housing
stability; 3) work with able bodied residents between the ages of 19 and
64 to train for employment, find employment and retain employment; 3)
work with families to support the involvement of young children in high
quality early childhood and youth development programs; 4) work with
youth and their families to support involvement in education and other
activities and programs that will pave the way for productive adulthood.
BASIC FUNCTION:
Incumbent
works to build relationships with resident families, facilitates the
creation of family development plans, and brokers services and supports
to help resident families reach their goals. Incumbents will be
client-centered, and work with a team of professionals, including case
managers from local government agencies and community-based
organizations, relocation and employment service providers, to
coordinate an array of effective services, supports and education to
assist families in reaching their self-sufficiency goals, and to ensure
successful transitioning of family members into new schools and to other
services and opportunities in the new neighborhood. Incumbent utilizes
knowledge of the San Francisco community to establish effective
relationships with businesses and social service organizations.
Incumbent provides case management and seamless service coordination by
closely and thoughtfully coordinating relationships among selected
service providers and partners. Incumbent provides supportive services
activities that promote economic self-sufficiency, enhance quality of
life and increase opportunities for residents as they prepare to live
successfully in a new, mixed-income development or move toward housing
self-sufficiency in the community.
TYPICAL DUTIES AND RESPONSIBILITIES:
1.
Undertakes aggressive and continuous outreach among residents to
communicate to them the opportunities associated with the program and to
promote participation by recruiting residents for appropriate
activities.
2. Advocates for residents and supports them in meeting
their goals and needs; acts as a liaison between residents and various
stakeholder agencies, service providers and partners with a
client-centered approach to delivering services and an outcomes-oriented
approach to achieving program goals.
3. Works with the Urban
Strategies Service Connector - Employment Specialist to emphasize
self-sufficiency through employment for persons between the ages of 19
and 64.
4. Learns, fully utilizes and prepares to support other staff
members in the use of electronic client tracking systems used for HOPE
SF projects.
5. Through home and office visits, meets regularly with
Hunters View residents and establish and maintain working relationships
with all heads-of-households and other adults between the ages of 19 and
64.
6. Performs initial and on-going assessment of each household.
7.
Develops realistic family and individual development plans for all
heads of households and all family members including children, youth and
elderly.
8. Maintains frequent contact with members of the assigned caseload.
9.
Works with other members of the HOPE SF staff, Urban Strategies, SFHA,
HSA, SFRA, OEWD and other community based service provider staff to
facilitate integrated services for residents and families as they
prepare for self-sufficiency.
10. Works with resident households to
encourage and support them in meeting the requirements of eligibility
and enrollment in appropriate human service programming.
11. Provides
ongoing coordination and follow-up of resource referrals for residents;
monitors and documents progress of residents enrolled in programs or
services.
12. Emphasizes the importance of high quality parenting
education programs and effective parenting skills for residents with
children.
13. Emphasizes the importance of high quality early
childhood development programs for residents with young children in
their households.
14. Emphasizes the importance of participation in
high quality youth development programs for residents with children or
youth in their households.
15. Participates in case conferences and
provide regular caseload progress reports to Urban Strategies and other
HOPE SF staff as needed.
16. Attends and schedules case conferences with agencies or partners residents as needed.
17. Participates in all required meetings, orientation, staff training, in-service and cross training opportunities.
MISCELLANEOUS:
Performs
other related duties, as needed. Subscribes to and supports the
purpose and mission of Urban Strategies, Inc. and the vision of HOPE SF.
OTHER REQUIRED SKILLS, ABILITIES AND EXPECTATIONS
1. Ability and commitment to handle privileged information in a professional and confidential manner.
2. Availability to attend evening and weekend functions as required.
3. Possession of a valid driver’s license and availability of automobile for use at work.
4. Ability to function as a team player in both internal and external relationships.
5. Good communication skills.
MINIMUM ACCEPTABLE QUALIFICATIONS:
Master’s
Degree in Social Work, Sociology, Psychology or related area, with a
