Safe Transition Program Coordinator

Centerforce provides education, information, and support services to people impacted by incarceration. We are seeking a dynamic individual to coordinate our Safe Transitions Program at San Quentin State Prison and California Medical Facility in Vacaville. The program evaluates adult males with mental illness and secures behavioral health and primary care services for them.

General Summary

The Program Coordinator will conduct mental health assessments of men returning to Alameda County, and connect them to health and mental health services. The coordinator will develop a pilot test program, protocols and tools for the successful performance of the program. The position will be based at Centerforce’s Yellow House in San Quentin Village, Marin County.

Job Description:

  • Program Administration: Oversee day-to-day administrative and program operations including coordinating logistics with the institution, developing, selecting and preparing materials, pilot testing materials, completing required reports, and maintaining ongoing communication with prison staff.
  • Service Delivery: Oversee the design, implementation, and evaluation of the Safe Transition Program. Recruit participants, conduct screenings, develop individualized transition plans and connect participants to health and mental health service providers.
  • Other Duties: As needed.

Qualifications:

  • Masters Degree in Social Work, or related field, two years of related experience in the Mental Health field working with the incarcerated, and/or with substance users.
  • Experience in counseling and group facilitation with high-risk populations, or an equivalent combination of education and experience.
  • The ability to work as part of a multidisciplinary team with people from diverse ethnic, cultural, economic, sexual orientation, and gender identification communities as well as the ability to work within the prison system.
  • We strongly prefer experience in providing service within a correctional system.
  • Proficiency with PC platform computers preferred.

All program staff are required to undergo training and security clearance by the California Department of Corrections which requires the following personal information in order to complete the application process: full name, California driver’s license #, social security #, and date of birth.

Compensation: This position is currently funded through April, 2013. The salary will depend upon experience. Other benefits are available.
