Dear All

 

I have a 43 year old reasonably fit Client with a recorded PSA of 5.4 in September 2022 and recent reading of 6.8.  He has been told he has an enlarged prostate.  He is on no treatment as he has been told that the PSA would be “higher and within range because he has an enlarged prostate”.  I have not heard of a correlation to PSA and size of prostate before is this correct?

 

I also want to confirm that a biopsy needs to be carried out for a Gleeson Score is that correct?

I feel unhappy that my client is not receiving any treatment or “watch and wait” with these readings.  Please advise.

Were he my client my take on this would be that for  a man under 60, a PSA level above 2.5 ng/mL is generally considered elevated. Given his  age (43) despite the presence of a large prostate , i would press him to get additional testing, such as a multiparametric MRI, to assess for potential prostate abnormalities. MRI scans are non-invasive and can help determine the necessity of a biopsy by identifying areas of concern within the prostate.
