Protectionof patient information is more important than ever before. You can be confident that we are doing everything we can at Hammond-Henry Hospital to keep your records safe. Your medical information is as important to us as our own.

There are a couple of ways to attain your medical records. For a condensed version, you can enroll for online access to My Patient Portal. Should you need a more in-depth copy of your medical records, you may contact our Health Information Management Department at 309.944.9108. One of our staff will be able to help attain your information stored at Hammond-Henry Hospital. Utilizing electronic medical records, we are able to quickly gather your records. You will need to complete an authorization for release of protected health information form and show a photo ID. We will then have the records available for pick-up or mail them to your home. Our offices are open Monday thru Friday 7 a.m. - 4:30 p.m.


Though email is very convenient, it can be unsafe to send important documents over email. To ensure patient identities and information stay safe we have made it our policy to not email records at this time.


No matter the nature of your records, you are always welcome to contact our Health Information Management Department, but if you wish you may contact the Imaging Department directly at 309.944-9135 for any imaging records.


Any information regarding advance directives, healthcare power of attorney, living will and the like are stored within Health Information Management. If you have any questions about setting up any of these types of documents, please contact our Social Services Department at 309.944-9170.


You can download the full dataset. Henry is former Dean of the ChicagoCollege of Performing Arts at Roosevelt University, formerPresident of the Leagueof American Orchestras, and former President of the Chicago Symphony Orchestra (you might enjoy thisinterview with himfrom his time at the CSO). He has beencollecting records for many decades. All areas of classical music arewell-represented in the collection, but it is particularly strong in early-and mid-twentieth-century operatic and vocal recordings, the conductorWilhelm Furtwngler, and several unjustly neglected composers andperformers who I'm sure you would agree deserve wider exposure ifonly I would name them here, which I won't because can you imagine thereader mail it would generate if the list didn't include someone'sfavorite? Oy vey. Let's not even go there.


I'm Henry's son Karl, and a lifelong beneficiary of the collection.Starting in the late 1980's, we began entering the records into acomputer database. After a multi-year cataloguing spree, the wholething was done, and Henry has kept it up to date ever since as heacquires new recordings. Unfortunately, the one area that stillhasn't made it into the catalogue is his opera aria recordings (discsgenerally devoted to one singer performing a variety of arias andsongs), which are a major portion of the collection. He's not rulingout all hope of getting those finished someday, but it is a formidabletask because each song on each disc must be entered by hand, and someof the discs hold ten or twenty arias.

Your medical information is secured and protected by HCH. To obtain a printed copy of your medical records, please download and complete the HIPAA-compliant Release of Information Authorization Form. Submit the completed form, along with a copy of your valid photo ID, via one of these methods.

HJAHC receives HHS funding and has federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.


Henry Ford Health is dedicated to collecting, maintaining, and providing quality health information. We are committed to assuring that accurate patient information is available to our customers in an efficient manner, while protecting the confidentiality and security of that information.


Henry Ford Health MyChart allows you to review portions of your medical/surgical history including lab results, test results, current immunizations, provider notes. It is free to use, completely secure, and available 24 hours a day.


You may also request the remaining portions of your medical records through MyChart. Please allow up to 3-5 business days from the submission of your request, for your records to be processed and released.


Medical records can be released to the patient or anyone that the patient authorizes by completing the medical records authorization. It may take 7-10 business days to process your request. There may be a fee charged for your medical records.


You can sign up for Henry Ford MyChart which allows you to review your medical history (lab, test results, current immunizations, preventive care and medical/surgical history) on a secure line, free to use and available 24 hours per day.


Beginning April 29, 2020 Henry Ford Health MyChart offers a free and secure on-line access for patients to receive their Epic medical records. The requested records will be uploaded to the patients MyChart account.


If the patient is not signing, a copy of the Letters of Authority as Legal Guardian, Medical Power of Attorney, or Personal Representative must accompany the form. The form must be completed in full.


The Virginia Freedom of Information Act (FOIA), located 2.2-3700 et seq. of the Code of Virginia, guarantees citizens of the Commonwealth and representatives of the media access to public records held by public bodies, public officials, and public employees. FOIA provides that, with some specific exemptions and exceptions, all meetings of public bodies shall be open to the public and all public records open for public inspection. All public meetings and records are presumed open unless an exemption is properly invoked.


A public record is any writing or recording -- regardless of whether it is a paper record, an electronic file, an audio or video recording, or any other format -- that is prepared or owned by, or in the possession of a public body or its officers, employees or agents in the transaction of public business. All public records are presumed to be open, and may only be withheld if a specific, statutory exemption applies.


You may request records by U.S. Mail, fax, e-mail, in person, or over the phone. FOIA does not require that your request be in writing, nor do you need to specifically state that you are requesting records under FOIA. However, you must provide your name and legal address. It is recommended that prior to submitting a FOIA request for records you should review the remainder of the County's website and to use the site's search function. In many instances, the requester may find the information or records that he or she is looking for online.


Your request must identify the records you are seeking with "reasonable specificity." It does not refer to or limit the volume or number of records that you are requesting; instead, it requires that you be specific enough so that Henry County can identify and locate the records that you are seeking.


Your request must ask for existing records or documents. FOIA gives you a right to inspect or copy records; it does not apply to a situation where you are asking general questions about the work of Henry County, nor does it require Henry County to create a record that does not exist.


If Henry County has questions about your request, please cooperate with staff's efforts to clarify the type of records that you are seeking, or to attempt to reach a reasonable agreement about a response to a large request. Making a FOIA request is not an adversarial process, but Henry County may need to discuss your request with you to ensure that we understand what records you are seeking.
