Here is how to change it:
"Tools" Menu > "Preferences" entry > "Editor" page > "Advanced settings" tab > "Edit template for new modules" button

How to change header? Where I can do this?

I'm afraid it's not implemented and it may not be possible (if I understand correctly your request) because the filename is still unknown when creating the file contents from the Template.
