How do I get the X-boxes in a clickable quiz?

Hi all!  

I am making a clickable quiz where each hint has several correct answers to click.  How do I make it look like this one where it has the checkboxes and the ratio of how many answers and how many are right so far?

You can just copy and paste symbols into your Hint box. 

This is one good site for that: http://ss64.com/symbols.html

Be sure to add a <br> (paragraph return) before your checkboxes so they will show up on the next line.
