Hello all :)

as far as i know the business card is compulsory for all HICOM, HOD and
AHOD, while optional for other committee members.

Details:
1. Business card design (Wordings and logo will be slightly squeezed
inwards because the vendor is unable to print with them being too close to
the edge):

2. 100 pcs of business card per box.

3. Price: RM 17 (with a minimum order for 18 persons, if we can get more
than this number i wil try to negotiate the price, any extra amount will be
refunded.)

4. Order: Please ignore all the previous spreadsheets and forms, and fill
in this *new* spreadsheet dedicated to only the business card.

5. Payment: Immediately after filling in the spreadsheet, you may transfer
the payment (RM17 each box) to my CIMB account. (account holder name: Lek
Chun Hou ; account number: 08030002878202). Please fill in my email address
(namele...@gmail.com) so that i will be notified, also send me a copy of
the receipt to namele...@gmail.com.

Deadline: Next monday, 15 Oct 2012.

NOTE TO ALL

how you fill in the spreadsheet is how the wordings on your business
card will appear to be (capital/small letters). please double check
and make sure the info is accurate. (don't miss out your department)

exception for mobile phone number, a '+' sign and 'space' after +60
will be compulsory. (eg: +60 10 376 6724 )
