Impact Concussion Testing information

Hey Sparks-
I am attaching a form for ImPact testing that is available to all of our athletes here at Highland.
This testing is used for data in regards to Concussions and in determining participation eligibility after a concussion has occurred.
Here is information regarding the testing dates and times: (Please note, the Saturday AM is prior to the start of the meet on Saturday and could be an option for some). On Tuesday, 9/9, SPA has a swim meet so please take this into consideration.  For Highland, if you choose to test on Tuesday, you will be excused from this portion of practice (does not take all of practice, so attendance expected after).

Due to the limited computers, we would need to have the students “RSVP” to me at ryan.w....@healthpartners.com with their top 2 choices from the following list:
