Hi Admin,
Because we do not yet have the template for the End of the Year Evaluation, I wanted to give you all a checklist of possible information you may need to provide in the eval. I wanted to give this checklist to you early because this is information you may need to collect during site/ask PAs and PTs for.
The first page is general information either all of us or specific admin members need to provide that you might not have had to provide in past evaluations.
The second page are just a heads up of what to keep in mind for for the evaluation.
The third page is the SAPEP information we need to provide so the university knows the demographic we're serving. I've divided it up into who needs to provide which info.
I'll give you all a final template once I receive it from the SIAC.
Again, a lot of this information may take a while to collect so please open up the template and take note of what you need to collect.
