Lost User Name & Priviledges

Hi

HELP please
Changed my laptop now can't sign in as my long used user name and have lost my priviledges as well. All happened with the change of laptop and 3 days waiting.

I've had to change name to use SLSK from Golden Hips of Sandie Shaw to Golden Hips of Sandie Shore

Nir can I get my User name back as well as the priviledges I paid for. How do I do this
