How to set up a new account?

When opening QT, I suddenly received a message:

"Login failed due to a wrong username/password combination. If you haven't logged in in a while, it's possible your username was recycled and taken by another user. Please try logging in with a different username and password"

But how do I set up a new user name and PW?
