Hi Health Care for the Homeless Collaborative (HCHC), and CHOW Team- This is a reminder to send in your referral forms for this Friday's anonymous Integrated Client Support Conference meeting (12:45 -1:55pm at NAMI, 1330 North Dutton in Santa Rosa). I've not yet received any referral forms, though many of you had indicated you had clients or patients who could benefit from this conferencing. I've attached the referral form again for your convenience. Please complete and submit one form ASAP (by Thursday at the very latest, earlier if possible) for each patient/client whose situation you would like to share at our conference on Friday. We are conferencing anonymously for this session (i.e. not sharing client/patient names or specifically identifying information).

I know that some of you have a hard time getting away from your hospital or clinic setting for this meeting. We're therefore setting up a Skype connection for any of you who could join our conferencing that way. We will have a laptop with an internet connection and Skype in our meeting room, but will have to know that you are planning to present a client situation, since I think we can only connect with one person at a time this way. That means that you have to be using a computer that also has Skype software. It is a free download, and takes only a short time to load. If we know you would like to participate, we can contact you when it is your turn to present. FYI, we tried setting up a speakerphone connection, but there is no active phone jack in that room.

Of course our preference is to have you physically present at the Client Support Conference, to help with other client needs, so if it's possible to make that happen we appreciate it. Please also send in your form if you have a client situation to present in person. The form has been revised based on input from CHOW and the HCHC, but it can be revised further for future conferences if you have additional input once you use it.

Since we're focusing on client conferences, we're not sending a meeting agenda this time. I hope to see many of you this Friday, and possibly connect via Skype with others.
