Any idea what is the issue below, and how to correct this ?

What is the output of onie-sysinfo command? When did it happen? Did you changed something with ONIE image?

We found the issue, for some reason a couple of bytes in the EEPROM TLV were wrong, after writing correct values, the error was resolved.
