Hi President!
3 quarters already pass since we start our term. 

Previously, we used SOLA as the quarterly reporting of each entity. Then, when we start the term, we try to evolved the SOLA and change it into BSC but there are lot of things need to improve from the tools. Now, based on the insight from all of you guys, we try to improve it and make it simpler, where we call it now, the "Organization Healthiness Report" where consist of several indicators that measure the healthiness of our organization from Internal Process, Financial, and Customer perspective. (you can find out the detail on the "APPENDIX 14 - Organization Health Indicators" of National Compendium (Legislated 27 January 2016), page 91-92 link: tiny.cc/AIESECIndoCompeJan2016)

Now, to assess the healthiness, please fill the "Organization Healthiness Report" on the attachment below.
In this report, it will not only measure the main indicators of each perspective that already mention above, but also there are several indicators that will assessed to understand the process to achieve the main indicators.

What do you need to submit?
With the template below, make two copies and please submit 2 reports of:
1. Semester 1 (Q1+Q2) report (except financial perspective)
2. Quarter 3 report

The deadline will be on April 15th, 2016 22.00 GMT+7

This report/tool will not directly measure your entity is healthy or not, since we don't decide the minimum score of the healthiness yet.
For example, your conversion rate of Raise to Match is 80%, but it doesn't mean this rate is classified as healthy or not. After several report, we can see the progress in all entity and then we can decide the minimum score. 
Because of that reason, your report will be really useful to develop this tools and also will become the documentation of your term that will be useful for entity planning & analysis next and several terms ahead.

**Please be aware that not submitting it on time, meaning that you won't be fulfill member criteria.
And also if the data is not reliable, it'll be invalid as well.

BE REALLY AWARE THAT FAILURE IN SUBMISSION CAUSED BY TECHNICAL REASONS/LAPTOP FAILURE/WEBSITE PROBLEMS/ILLNESS WON'T BE ACCEPTED and WILL RESUL IN YOUR ENTITY NOT BEING ABLE TO GET MEMBERSHIP.

Regarding the submission of the "Organization Healthiness Report", there are several confusion in filling the internal process perspective because the report has to be per quarter but the operation is per peak (winter and summer),

for example in Q1+Q2:
raise 50
match 30
Realization 2

so the conversion rate will be 
% raise to match = 30/50*100% = 60%
% match to realize = 2/30*100% = 6.67%
in Q3
Raise 0
Match 3
Realization 33

so the conversion rate will be 
% raise to match = 3/0 (ganti pembagi nya jadi 1)*100% = 300%
% match to realize = 33/2*100% = 1650%

Because of it maybe some of you will confuse about the conversion rate. For that reason, here I attach the new template of the Organization Healthiness Report where you don't need to input the conversion rate but just directly input the number (# of register, # of pass interview, dan seterusnya) so it will not make you confuse about the conversion.
