The Care Coordination Progam is a new DOH program that's kind of in its pilot stage, so it's ever-changing but also exciting to be part of.  It's a very diverse position with a balance of direct patient care, program management and supervision of field-based staff.  Some clinical work though not ongoing psychotherapy.  We have two openings - one in Harlem and one in the Bronx (not too far in).  Bilingual a plus but not required.  Anyone who is interested can email me at ahest...@institute2000.org if they have questions, or can send cover letters and resumes directly to program director Liz Bodman at ebo...@institute2000.org.

Care Coordinator job description:
 
Works as part of interdisciplinary team to help HIV positive patients address personal and environmental difficulties which negatively affect treatment adherence. Care Coordinators will assist in recruiting eligible patients for enrollment in the Care Coordination Program. Care Coordinators are responsible for completing intake and assessments, developing and maintaining quarterly service plans and reassessments, organizing and leading quarterly case conferences, and in-clinic follow up to all program participants. In addition, Care Coordinators provide administrative oversight to Patient Navigators, including but not limited to monthly chart reviews and administrative supervision, and oversight of day to day functioning. Additional responsibilities include the provision of health education, and conducting home visits when no alternative coverage is available. Care Coordinators will screen for benefit eligibility and address patient’s individual needs as they pertain to treatment adherence and consistent medical follow up. Meets regulatory and annual health assessment requirements.
EDUCATION REQUIRED
  • Masters Degree in Social Work.
LICENSES/CERTIFICATIONS REQUIRED
  • Knowledge of HIV programs and services in the New York City area
  • Ability to write professional reports and correspondence.
  • Ability to work independently and as part of a team.
  • Knowledge of health care and health care applications.
  • Ability to identify resources and utilize problem solving skills in order to meet patient’s individual needs.
  • Excellent interpersonal skills.
  • Excellent time management and organizational skills.
  • Basic knowledge of computers required.
  • Managerial experience a plus.
  • Bi-lingual, Spanish preferred.
