Requestsfor vital records may be submitted to the IDPH Division of Vital Records via regular mail. Be advised that the processing time for requests is approximately 12 weeks from the time your paperwork is received. You will not receive an update from IDPH during the processing time.

In order to request a copy of a birth certificate you must complete the form Application for Search of Birth Record Files. To learn more, see FORMS located in the right-hand column. Completely fill out the form. Submit it with the following information:


A valid government issued photo identification (ID) that must be readable and is not expired/out of date (if an ID is not provided, unreadable, or expired, the request will be returned unprocessed).


For $15 for the first copy of a birth certificate and $2 for each additional copy of the same certificate requested at the same time, you will obtain a birth certificate with information collected at the time of your birth (information has varied throughout the years).


Only individuals with specific qualifying relationships to the subject of the record can receive a birth certificate or Heirloom birth certificate. Identity and proof of qualifying relationship documentation is required.


Birth certificates serve as official documents printed on certified security paper and used for legal purposes to prove a life event. Uses include proving identity, citizenship, and relationship to others in order to get a passport or driver license, adding a child to health insurance, enrolling a child in school, or obtaining Social Security or other benefits and services.


It depends on the way you order and the shipping method you select. The times listed below start after we receive full payment and all required documentation. Missing information or documentation will delay orders.


If you have not submitted a request for a birth certificate by placing an order online, phone, mail, or in-person and submitted identification and proof of eligibility documentation, you will not receive a birth certificate.


New parents complete a Washington State Birth Filing Form at the hospital or birth center after delivery, not an application to order a birth certificate. The first birth certificate issued for a birth is NOT free and hospitals and birth centers cannot order a birth certificate on your behalf. The hospital may have provided you with your local county health department birth certificate order form, which requires a fee.


The Governor signs each Washington State Heirloom birth certificate individually. For this reason, it can take up to 8 - 10 weeks for you to receive an Heirloom Birth Certificate after we receive your order.


If you want to use a certified document in another country, you may need an apostille from the Secretary of State. For example, a birth certificate issued in the state of Washington that you want to use in another country, may require an apostille.


The birth registration team is responsible for registering live births filed by hospitals, midwives, or other individuals who deliver in Utah. A date of registration is assigned for each live birth certificate and any supporting documentation that is needed to register the birth certificate such as a paternity declaration. The registration team registers both current birth events and events that are older than one calendar year and have not yet been registered with the Office of Vital Records and Statistics (OVRS).


All live births filed with the Office of Vital Records and Statistic (OVRS) for individuals one calendar year or older shall be filed as delayed. Even if you have never obtained a certified copy of your birth certificate, or know that you do not have a birth certificate on file, you will need to start the process by completing an application for a certificate of live birth. Order a Birth Certificate online or print a paper application. If applying by paper application follow instructions provided on the application, and along with picture ID and the required fee, mail the form into OVRS. OVRS will contact the applicant for further information and steps to be taken if a delayed birth certificate needs to be filed.


Utah law allows for the registration of early-term stillbirth deliveries for pregnancies between 16 - 19 weeks gestation as calculated from date of last normal menses period to the date of delivery. The request form requires a Utah licensed physician to sign. Follow instructions provided on the application, and along with picture ID and the required fee, mail the form into the Office of Vital Records and Statistics. The purpose of the Texas Birth Registrar Certification is to improve birth registration in Texas by standardizing data collection and entry. Improving birth registration is critical to make sure Texans receive valid birth certificates, and that the data represents their health.


Birth records provide important public health information. The data must be accurate, complete, and measurable. Statistical data impacts the general health of Texas because it relates directly to research, public health programs, medical practices, and funding.


The Vital Events Data Management team develops, analyzes, and distributes public health data from records of vital events. The team also replies to statistical data requests and develops the Texas Vital Statistics Annual Report.

After you finish using the online 5-in-1 newborn bundle, mail the additional form and required evidence to the Office of the Registrar General to complete the registration. All parents must be present when using the online 5-in-1 newborn bundle in order to agree to the single name.


You can register the birth of a baby with a last name that is different from the parent(s) last name using the online 5-in-1 newborn bundle. All parents must be present at the time of registration to agree to it.


If the birth involves a surrogate, make sure that you have the following forms completed, signed and commissioned before starting the online registration. You will need to mail them to the Office of the Registrar General to complete the registration.


A record cannot be identified as available prior to purchase because under State statute (IC 16-37-1-11 and IC 37-1-11.5), the fee associated with a birth certificate is for the IDOH Vital Records office to search the records and determine if a record is on file. The search fee includes one certified copy of the certificate if the record is found; search fees are non-refundable.


Indiana does not issue birth certificates for individuals born in another state. Requests must be made directly to the vital records office in the state where the birth occurred. To file a home birth within 12 months at the local health department, documentation will be required. Evidence is commonly requested for proof of residence within the county or jurisdiction where the birth is being registered; however, other documentary evidence may be required depending on the health department.


For a delayed birth to be registered, proper documentation, completed forms, and associated payment is required. Documentation requirements vary depending on the individual's age at the time of registration.


The Oklahoma State Department of Health Division of Vital Records is responsible for registering every birth which occurs in the State of Oklahoma as well as preserving, amending and issuing certified copies of those records in accordance with state law.
