GEN-DE Newbie needs help - Wurzburg Baptismal Register

I need some help reading old German script. My high school German just isn't up to the task yet - this is the first set of records I've had to read.

The record I'm trying to translate first is a baptismal register entry from the Diocese of Wurzburg.

I've also posted my original tif scans so you can zoom in, if you wish.

I know the column headings are probably standard, but I just can't make them out on my copy. If someone could help me with those, I would greatly appreciate it. Help on the actual entry would be a bonus -- I think I have most of it except the last column and the religion(?) columns.

The headings are, unfortunately, cut off at the top, so only the second line is
