Iam concerned. After having a Upper Bleph a year ago found a wonderful Plastic Surgeon who gave me my first BOTOX on my forehead. . Now my Plastic Surgeons practice is using a Nurse Injector. What are P.S. thoughts on Nurse Injectors. My next is injection will be very important-My daughter's wedding. I always thought it is better to see a board certified Plastic Surgeon or Dermatologist.

Yes, I believe surgeon injectors are better than any RN injector but there are some very well trained RN's who can do it well and if they adhere to strict protocols and stay away from your upper lid muscles, your outcome should be a good one. I am assuming this nurse injector was well trained by the surgeon who employs her since she is a direct reflection of his practice.


Like Dr. Wong, surgeons have very extensive training in all aspects of facial anatomy and aesthetics and are more experienced injectors. Simply tell the office you would like the surgeon to do your injections if you have already had a good experience. Congratulations on your daughter's wedding.


I would recommend you call the office and let them know your concerns and ask that your surgeon preform your next injection for you. You may also ask to meet the nurse injector at that visit. Likely the RN is well trained, but since she/he has not treated you before the technique, dosage and placement of the Botox may be different than your surgeon. Different isn't necessarily bad, but given the stakes of this treatment, best not take the chance.

If you feel more comfortable with your surgeon performing your injection, by all means, do so--just tell the front desk people who you want to see. I will say that we have a very experienced nurse injector in our practice. She receives rave reviews from our patients and has a busy injection practice. She is well trained and exercises good judgement which keep her patients safe, happy and returning with other patients as well.


For me, Botox is simply one of the methods we use to improve the skin and face. It must be used considering all the other factors impacting the area. Therefore, I believe it is always best to have the physician who understands all these factors do the injections, or, at least, evaluate you first and carefully direct what is done.


Not every surgeon is a conscientious injector and many nurse injectors are quite talented. In my own practice I prefer to perform all injections but typically in offices where the surgeon has delegated this treatment the nurses will be well trained and experienced. In this special instance your surgeon will likely honor your request to have him/her perform the treatment. Best of Luck Dr Harrell


1.) Not that familiar with ACFP, is this something that works well and last a long time or something that requires constant tweaking and expensive to maintain? Is it doing more than manually learning for you in the climb?


2.) I have had GAMI injectors and do like them a lot, but not familiar with CTI. Is Continental simply riding on their coat tails and created there own tunes injectors? If so does it matter which one to go with?


The difference in fuel flows is obviously higher at higher power settings. This is where the possibility of reduced CHTs completes the show, as that feature may allow use of higher power without high CHT.


When going LOP there is a noticeable loss of airspeed as the power is simply reduced. If one notes this drop, say 10 kts, and were to merely reduce throttle when ROP to achieve the same IAS the numbers for fuel flow get much closer together. One must also factor in how ROP one operates. If you typically cruise at less than 65% power, the smart guys I know have been pretty unamious in saying that you can not hurt the engine with the mixture control. Thus operating much closer to peak EGT is acceptable if your engine is running smoothly. At higher powers it is very important to operate richer than the typical OEM recommendation of 50dF rich of the first cylinder to peak. I think the recommendation is at least 100dF rich. All of this affects the fuel flow comparison, thus creating another aviation diaper situation (it depends).


We got the TCM tuned injectors on last reman. I had experience with GAMI in two previous engines. Frankly the TCM injectors work just as well for me. The downside is that you get what you get. There's no tweaking by sending them back like you can do with GAMI. In my case the ones I got are as good as I could hope for in peak EGT spread.


I would suggest not getting the altitude compensating fuel pump. It requires a flight test each annual to maintain the auto leaning schedule. The regular pump does not need this procedure. As long as you are comfortable with the leaning procedure the normal pump is the best bet.

The Continental nozzles are now the latest version and work very well. As someone else stated, you get no additional support if you have 1 cylinder running hot or cold with the Continental nozzles.

With GAMI nozzles, they will provide support by replacing nozzles as needed to get the proper fuel flow balanced to each cylinder.


I just received a factory reman from Continental. In the ordering process I asked if I could get the engine without injectors and they could subtract the cost (which I could then use to purchase GAMIs). They said they couldn't do this but they said the engine has CTIs which are comparable. Opinions vary on this in the community (I've heard one qualified SME tell me that the CTIs are very comparable). They didn't offer a lesser or other option. I've also read about some with the CTIs have gami spreads comparable to gami injectors so figured I could check the gami spread with my engine monitor to see if I needed to get gami's after the fact.


Did they offer CTIs as a separate option or just tell you they came with it? With mine, it wasn't an option. If they offered it as a separate priced option I'd be interested to see if they've got a different story.


The option was for the ACFP which I am still not sure if thats good or not. I know there was a service bulletin to remove the ACFP (OPTIONAL) but for twins (MANDATORY). Not sure if that was rescinded or not.

Indeed I was definitely getting balanced injectors to lower CHT and extend engine life, not just to lower the fuel burn. I should've said so. But I don't know anybody personally with GAMIs in 470s to ask about the fuel savings aspect so thought I'd ask here. Typically I have cruised at 65% and occasionally 75%.


Wait, you can only use these alpha injectors if you are below 5M skill points? Then what am I as Omega supposed to do with these useless injectors? I mean, selling them on the market with this little demand is hardly more useful than outright trashing them.
