We have a provider-facing SMART on FHIR application that right now only operates in sandbox mode. To login, we used the test credentials published by Epic here:

It worked well, but then suddenly stopped. Now, both FHIR and FHIRTWO usernames return the following error message:
"User authentication is blocked for your account. Contact your system administrator to unblock your account."
Has anyone else experienced something similar? Any clue what can be causing this?
