MA Licensing

Hi everyone (and congrats!!!)

I was on the mass.gov site and noticed that there is a new version of
Form 1 and Form 2 that we need to fill out (much easier than the other
one - don't need to fill in the specific activity hours/week!) Form 1
needs to be filled out and mailed in within 30 days of starting your
supervised position.

Do we just send in the form, or is there some kind of "plan" that we have to type up and attach?

Anna,
I don't see anything on the form that states you need to submit a plan
for the new MA license Form 1 - if someone knows differently, please
post. (This form seems less cumbersome than ASHA's requirement).
It is stated, for those of us in substitute positions, that if your
supervisor changes, you must submit a form 2 to complete that portion
of the supervised practice year, and remit a new form 1 and form 2 for
each new supervisor.
