Has anyone prionted the "certificatio and membershi" form from ASHA that we are supposed to fill out before record check?  I can only find a form called "Application for Memebership without Certification".  Is that it?  It says something about only being for current members...

I think the whole portfolio time is 45 minutes but don't know how muct to allot to this. They never said how much of the intro to include...

pages, the rest of it is the KASA.
