For the highest page issue, I just put the index in page number order when I'm done to make sure there isn't a page number that's too large. If there is, I can quickly fix it by putting it in as-entered order to see what it was supposed to be.

And when page numbers seems wonky -- like one way out of order, I go to View>date created or date edited and sometimes view the record numbers. All that data helps me see where in the sequence of things this wonky page number occurred.
You question about the chapter overlap is trickier. Maybe all you can do is to regularly group on page number and put in the beginning chapter page # and the ending one. Then take a close look at the entries to see if they fit in that chapter?
