If all of your settings are maintained after the reinstall give me a call or send me an email and we'll try another thing to reset the settings back to default.

I have also attached 5 screen shots of various important panels in the DepthQ Control Panel.  For the most part they reflect the standard default settings.  However, on the stereo output tab, you will want "Horizontal Interlaced" instead of the default which is "OpenGL HW".
