There is no biometric services fee. Anyone may pay this fee from anywhere in the world, as long as they enter the correct first and last names, date of birth, and A-Number because this information will appear on your boarding foil. You can find this information on your Green Card, reentry permit or advance parole document. Whoever pays the fee must pay through our online payment system using a credit or debit card or U.S. bank account.
If you are the purchaser, fill out Form ST-125 and give it to the seller. You must give the seller a properly completed Form ST-125 within 90 days after the delivery of the property or performance of the service. Sales tax exemption certificates may also be issued and accepted electronically. See TSB-M-07(1)S, Electronic Resale and Exemption Documents for Sales and Compensating Use Taxes.
You may use Form ST-125 as a single-purchase certificate, or as a blanket certificate covering the first and subsequent purchases of the same general type of property or service from the same seller.
Form ST-125 can be used to make exempt purchases of property and services used predominantly (more than 50% of the time) either in farm production, in a commercial horse boarding operation, or in both. It can also be used to make exempt purchases of drugs or medicine for use on livestock and poultry used in farm production.

If your pet is new to Needham Animal hospital, please fill out a "New Pet Intake Form" as well as the boarding registration form. This form can be found on the main Clinic Forms page. Please fill out a separate boarding form for each pet staying with us. Please note that pets from the same family are NOT boarded together overnight but may stay together during business hours. In order to minimize the time it takes to get your pet checked into boarding, please fill out this form, at minimum, one business day in advance of your boarding reservation. This will save you time at drop off. You are welcome to fill out the form in person or immediately before arrival but please allow for additional time at drop off if you elect to do so.
I, the undersigned client, hereby give my consent for my pet (named above) to be boarded at Main Street Veterinary Clinic (MSVC) on the above mentioned dates. I acknowledge that I have received and read MSVC's notice on its lack of fire suppression and overnight staffing. I understand and acknowledge that MSVC is not equipped with an on-site fire suppression sprinkler system. I further understand and acknowledge that MSVC is unstaffed between the hours of 6pm and 7am during the week and most of the hours on the holidays and weekends. I understand that my pet will be left unattended during those times and during their boarding stay.
By signing this consent form, I agree to release MSVC, its owners, employees and agents from any and all liabilities, claims and/or expenses arising from my pet's boarding stay, including but not limited to injuries, illnesses or death.
I have read and understood the statements outlined in the notice and in this consent form and release. I voluntarily consent to my pet staying/boarding at MSVC and acknowledge that I am solely responsible for any consequences that might arise during their stay.
Medication while boarding: A medication charge will be added to your bill for any boarder that comes in with medication or has an exam with a presenting concern on the day of arrival and the doctor prescribes medication. If the pet requires medication for an acute problem after arrival, no charge will be added.
Bathing: All dogs staying for boarding will receive a complimentary bath, brushing, nail trim, and ear cleanse on the day of pick-up. If you do not want your dog to receive a bath, you may arrange an earlier pick-up time with our staff. Please confirm the pick-up time with our staff as hours may vary due to holidays or other factors.
You must pay a deposit of $35.00 at the time of making your reservation. This charge is one day of boarding at our lowest rate and will be a credit on your account. You must confirm your reservation within 48 hours of the first day of boarding. The deposit will also be forfeited if you do not cancel within 48 hours, or fail to show for boarding.
This industry group comprises establishments primarily engaged in operating rooming and boarding houses and similar facilities. These establishments provide temporary or longer-term accommodation, which, for the period of occupancy, may serve as a principal residence. These establishments may also provide complementary services, such as housekeeping, meals and laundry services.
If your cat is boarding when a dangerous storm is imminent, we will make every effort to contact you and give you the option of picking up your cat or releasing your cat to another family member or responsible party. Please leave contact information for this person below.
If we cannot contact you, or arrangements cannot be made to have your cat picked up, All Cats Hospital will take all possible precautions to care for your cat while it is boarding. Due to the unpredictability of these types of storms, if a storm mandatory evacuation is ordered, All Cats Hospital staff must evacuate. We will make sure your cat has plenty of water prior to evacuation, and we cannot guarantee the safety of your pet, nor can we be certain as to when we will be able to gain access to the boarding facility after the storm passes.
I have read the above information and understand that during the Hurricane Season in Florida (June 1 through December 1), I may be boarding my cat at my own risk in the case of unpredictable dangerous weather conditions that may occur during this time.
I further understand it is hospital policy that all annual vaccinations be up to date at the time of boarding. Furthermore, the hospital requires all boarding canines(s) receive an intranasal vaccination to help prevent the incidence and spread of kennel cough. These vaccination requirements reduce the risk to hospital employees and improve the general health condition of the boarding facilities.
Boat slips required to comply with F235.2 and boarding piers at boat launch ramps required to comply with F235.3 shall be on an accessible route. Accessible routes serving recreational boating facilities shall comply with Chapter 4 except as modified by 1003.2.
Direct connections to other facilities shall provide an accessible route complying with 404 from the point of connection to boarding platforms and all transportation system elements required to be accessible. Any elements provided to facilitate future direct connections shall be on an accessible route connecting boarding platforms and all transportation system elements required to be accessible.
Bus stop boarding and alighting areas shall provide a clear length of 96 inches (2440 mm) minimum, measured perpendicular to the curb or vehicle roadway edge, and a clear width of 60 inches (1525 mm) minimum, measured parallel to the vehicle roadway.
Parallel to the roadway, the slope of the bus stop boarding and alighting area shall be the same as the roadway, to the maximum extent practicable. Perpendicular to the roadway, the slope of the bus stop boarding and alighting area shall not be steeper than1:48.
Bus shelters shall provide a minimum clear floor or ground space complying with 305 entirely within the shelter. Bus shelters shall be connected by an accessibleroute complying with 402 to a boarding and alighting area complying with 810.2.
Lists of stations, routes and destinations served by the station which are located on boarding areas, platforms, or mezzanines shall comply with 703.5. At least one tactile sign identifying the specific station and complying with 703.2 shall be provided on each platform or boarding area. Signs covered by this requirement shall, to the maximum extent practicable, be placed in uniform locations within the system.
3. Where the accessible route serving a floating boarding pier or skid pier is located within a boat launch ramp, the portion of the accessible route located within the boat launch ramp shall not be required to comply with 405.
Boarding piers at boat launch ramps shall provide clear pier space 60 inches (1525 mm) wide minimum and shall extend the full length of the boarding pier. Every 10 feet (3050 mm) maximum of linear pier edge shall contain at least one continuous clear opening 60 inches (1525 mm) wide minimum.
** ATTENTION** Appointment must be made with reception prior to filling out this boarding form. Without scheduling your appointment, it is not guaranteed your pet will have a space in our boarding facility.
We understand that many of our boarders are on medications, and we are happy to accommodate their needs while boarding. Medications can be given up to twice a day at no charge. There will be a fee for administering insulin.

Some dogs exhibit signs of anxiety while boarding, such as vocalization, pacing, repetitive jumping or spinning, or an inability to settle. Trazodone is an anti-anxiety medication that can be given while your pet is here to help with these clinical signs.
