CTA requires all complains to be started by one person, and if there are other people also want to appeal, there are several ways:1. E-mail (email): pta...@otc-cta.gc.ca
1. Wait the outcome of first complain, the other passengers decided to draw a complaint or not individually
2. All other passengers also submit a complaint individually. (We'd better not be so, because if 110 people also submit, that means that each case will be delayed because of too many cases at once)
3. Other passengers authorite the person who file a complainant, and also started a complaint with that person (which is me).
Now, we use the 3rd methods. Attached is provided by tCTA, the standard authorization form . I have written on the top of my case number. You print out this sheet and fill in your information in part 2.
Part 1 of which is my (representer) information. Fill in your details to part 2 and signed. If a family has extra person, add all the other person's information written on the attached sheet of paper.
Note: Be sure to send this form by July 8 and make sure CTA received it. do not send to me. As follows:In the subject line, you should include: Authorization form RE: Canadian Transportation Agency Case No. :13-03435
2. Fax (Fax): 819-953-5686
If CTA doesnt receive your authorization form before the closing date July 8, you will need to re-start an individual case.
In addition, to ensure that everyone received this message, please send me an email as soon as possible to confirm you have recieved this form. Please do it by on time and remember to sent directly to the CTA instead of me. Otherwise it is invalid.
