Hi team,
I made quick form for note-takers of our upcoming usability test.

Sorry for my confusion btw data logging and note taking in last pilot test. 
Basically, I made separate page for individual tasks, presenting details of each tasks and leaving area below for note taking. 
If you have better suggestion, please let me know.

I referred back to the notes and logs from the in-class usability testing. I still can see the benefit of the form of logs, especially for the "user comments", "errors", "critical incidents", and "ask for help". It just doesn't need to be recorded in so much detail. I think if we are going to type in free form, we can still add the type of the notes (e.g. CU =comment by user, E= Error, != Critical Incident, H = Asked for Help, S = Success, F = Fail, Q = Quit/Ended the task) to help us record meaningful notes for us to reflect on. Besides that, I'm still thinking adding the timestamp to help us refer back to the video. Don't know if that is realistic or not though. 

Just a small discussion, if you guys have some ideas, please bring up on the table to discuss.

I would say table form is preferred as it offers a frame to follow and better readability later for both computer notes and hand-writing notes. Also I think from what we discussed last time: the content being jot down should be notes-centered, instead of timestamp-centered. Just saying either way will do, but in the table all our notes would be more unified as documentation. Including the tasks helps the observer to follow the task flow, so it is a good call!

I inserted page break between the tasks, so whole page can be act as the 'form' of notes, decided by each tasks.
I personally do not like tables, since it significantly limits the flexibility of adding / subtracting contents. Especially when taking notes by computer, adding/deleting rows takes much time.
I do agree type of the notes (i.e. CU, E, I …) may help us to categorize our note, as long as it does not interfere the note taking.
time stamp, since the note is already divided by the tasks, i don't think we would need additional time stamp for each notes. Also, it keeps note takers too busy hh.

any thoughts from others?
