I think that it would be appropriate to have a form for this. Of course, everyone can send you some basic information that you are probably looking for (like name, phone number, address, email and shevet) but there might be other details that you are looking for and there might be optional items in addition to the core items that you want. It would also be appropriate to indicate how you would want to use this information (and just as important is how you would not use this information, e.g. for fundraising).
