I will suggest call chase. It looks real but if u have not visited cvs at all then its definitely fake

Thanks everyone for your help. I called the Chase customer service instead of replying to the email. They told me that in addition to this declined transaction in SC, another transaction was done yesterday. So, somebody had made a copy of my credit card!!! They have cancelled my current credit card and are going to send me a new one by overnight mail service.
