Full Time Social Worker (MSW Required)

Salary Range: $101,800/year to $152,721/year

The Community Resource and Referral Center (CRRC) and the Health Care for Homeless Veterans (HCHV) incumbent within the San Francisco VA Health Care System (SFVAHCS) identifies and engages Veterans who are experiencing homelessness, marginally housed and/or at risk of homelessness, and/or who underutilize VA services. The incumbent provides Veterans with a range of social work services, including assessment and referrals, clinical treatment, short-term clinical case management, advocacy and service coordination, and linkage with VA and community services. This is accomplished in concert with members of a multidisciplinary team.

 

The incumbent possesses knowledge of and is sensitive to the needs of Veterans experiencing homelessness. The incumbent has knowledge and experience implementing medical, mental health, and substance abuse oriented treatment modalities. Is able to complete a comprehensive psychosocial assessment to identify strengths, needs, social support systems, disabilities, and barriers to accessing housing and treatment options. Has general knowledge of Veteran’s benefits and services related to special programs, service-connected compensation, non-service-connected pensions, and benefits that are available on the federal and state levels, and through city/county social services.

 

The incumbent is aware of the specific challenges related to working with Veterans with co-morbid physical, mental health and substance abuse diagnoses, and is able to effectively work with this Veteran population.

 

The incumbent is familiar with community resources and can assist with linking Veterans to appropriate community service providers. The incumbent is responsible for collaborating with community providers to increase resources for Veterans working within HCHV/CRRC programs. The goal is to increase stability in Veterans’ lives and assist Veterans in returning to independent, healthy, productive lifestyles. The incumbent is available to work evening hours, as needed, to meet the needs of Veterans that are not able to be seen during regular business hours of operation or for evening groups.
