I ended up building a Windows 98 SE laptop, and installing on that the TWAIN32 driver for DSC4xx and the Adaptec (4.71, I believe) ASPI driver. With that, I was able to use Photoshop 5 to launch the TWAIN driver and connect to the camera over an Adaptec 1460 SlimSCSI card and cable.

It is possible that the driver installation worked. Did you try connecting the microscope through the USB cable to the computer and look in the Windows Device Manager to see if the serial port is listed there?

Yes I managed to get the cable on the left working (on windows 7 64 bit) with right driver. It was a right fiddle as you have to try and delete the old drivers.
I think I got the right driver from here

Firstly, in order to fix the problems, right-click the My Computer/Computer icon, select Manage and open Device Manager. Expand the Universal Serial Bus controllers list, right-click on the driver and press Uninstall. Reboot the PC, connect the station using the USB cable, press Scan for hardware changes icon from Device Manager and wait for Windows to retrieve the drivers. If your Leica product is using a Serial connection, you will need to use a Serial to USB adapter. After that, use the instructions from the official web page and download and install Leica Geo Office Tools. Next, launch the software and select Data Exchange Manager from the Tools menu:
