Would it be possible to get this form attached just as a normal pdf so it can be printed, completed and scanned?  It's wanting me to download and install apps and authorize it to share my info in order to open it.

I see it's also on the drive, but I can't use google drive.  A regular old style pdf would be perfect.  Thanks for your help!

Adobe Acrobat Reader DC works just fine to fill it out once you download it to your local...

Yes.  If you have access to google docs or you are able to download a special acrobat reader you can use the original link or the web page that you linked Chandra.  The problem is that I can't download a program to my work computer or on the public computer I use for such things and my printer at home is broken so I can't print it out and fill it out and send it back unless it is a normal pdf or word attachment.  I went to UPS to use their computers to use reader and print the document, but they would'nt let me download the program on to their computer either.

At the build party before last it came up again that it's hard for some people to access google docs, and that people weren't getting mail chimp emails.  If we'd like to be as inclusive as possible I'd send ordinary email with attachments like regular pdf, jpeg, doc etc that multiple people can use. 

It's just food for thought.

I'm a luddite and I'm lucky to even get to my emails in a timely manner. I can print your form at my office and bring it to the build if you want, this goes for anyone.

Everyone is free to fill out their medical form and bring it to the playa.  Please bring two copies: one for our camp's medical file and one to carry on your person (so if you are "found down" the medical team can figure out what to do with you and not have to take a random stab at it).
