

2015 Live Well San Diego Public Health Champion Awards
Call For Nominations!
The County of San Diego Health and Human Services Agency (HHSA), Division of Public Health Services is proud to announce the 14th
Live Well San Diego Annual Public Health Champion Awards. These awards recognize the extraordinary achievements, by individuals and organizations, in areas of public health services that protect and promote the health of San Diego County residents. The
awards were created to recognize the role of public/private collaboration in advancing optimal health and wellness in the County’s diverse populations.
This year’s theme award will recognize achievements related to National Public Health Week‘s (NPHW) focus on creating strong public health systems across the country. The 2015 NPHW
theme is “Healthiest Nation 2030: Let’s Make America the Healthiest Nation in One Generation.”
Public Health Champions Awards is a program of the County’s Live Well San Diego initiative, a long-term strategic vision for improving health and wellness of all San Diegans.
The Awards Program will be held at the County Administration Center on April 10, 2015, in conjunction with National Public Health Week.
This year the categories are as follows:
1. Public Health Champion Awards – Theme:
“Healthiest Nation 2030: Let’s Make America the Healthiest Nation in One Generation”
2. Public Health Champion Award – Individual
3. Public Health Champion Award – Organization
4. Public Health Champion Award – Regions
5. Public Health Champion Awards – Aging and Independence Services

March 10, 2015 Final Awardees Notified
April 10, 2015 Public Health Champions Awards Ceremony
